The U.S. FDA Approved Isembyld (apitegromab-mstn) for SMA
On September 11, 2026, the U.S. Food and Drug Administration approved Isembyld, also known as apitegromab-mstn, as an injection to treat SMA in adults and pediatric patients 2 years of age and older who are currently receiving an SMN2-targeted treatment. The FDA described Isembyld as the first approved therapy for SMA that directly targets muscle loss, working alongside existing treatments to address an important part of the disease that many people in the SMA community continue to face.
Most existing SMA treatments focus on increasing survival motor neuron protein through SMN-targeted approaches, while Isembyld takes a different path by targeting muscle loss. The FDA explains that SMA is caused by a faulty SMN1 gene that does not produce enough protein needed for motor neuron survival, and although existing SMN2-targeted therapies have improved outcomes for many patients, some individuals still experience major motor limitations. In a 52-week randomized, double-blind, placebo-controlled trial of 188 participants with SMA between ages 2 and 21 who were already receiving an approved SMN2-targeted treatment, Isembyld was studied for its effect on motor function; among patients ages 2 to 12, those receiving Isembyld 10 mg/kg showed significant improvement compared with placebo, and 34.2% had a clinically meaningful improvement compared with 13.5% on placebo.
