The True Cost of Spinraza for SMA?
Spinraza, also known as nusinersen, was the first disease-modifying treatment approved for SMA in the United States, but its price also makes it one of the most expensive long-term medications many families will ever encounter. Following the 2026 approval of high-dose Spinraza, Biogen priced the 28 mg vial at approximately $152,000 and the 50 mg loading-dose vial at approximately $271,000. The 12 mg formulation also carries a list price of roughly $152,000 per dose. These figures represent list prices before insurance negotiations, discounts, assistance programs, or other adjustments and therefore should not be confused with what an individual patient ultimately pays.
The true cost of Spinraza becomes clearer when the ongoing dosing schedule is considered. The traditional 12 mg regimen requires four loading doses followed by a maintenance dose every four months, while the newer high-dose regimen begins with two 50 mg doses given 14 days apart and continues with 28 mg maintenance doses every four months. At current list prices, the medication alone can total hundreds of thousands of dollars annually, and the financial picture may also include lumbar puncture procedures, imaging guidance for complex spines, hospital or facility charges, laboratory monitoring, specialist appointments, transportation, and caregiver-related expenses. Insurance coverage can therefore be critical. Cure SMA reports that insurance denials remain a significant issue for the community and can result in treatment delays, financial stress, and difficulty accessing specialist care.
