Living with SMA: The Reality of Muscle Weakness and Fatigue

Living with Spinal Muscular Atrophy, also known as SMA, often means navigating a level of muscle weakness and fatigue that other people may never fully see. SMA affects the motor neurons responsible for voluntary movement, causing muscles to become progressively weaker over time. Everyday activities such as lifting an arm, holding up the head, sitting upright, reaching for an object, transferring, eating, writing, or operating a wheelchair can require significantly more effort than they would for someone without SMA. Because proximal muscles around the shoulders, hips, trunk, and neck are commonly affected, even maintaining posture can consume energy. This is why SMA muscle weakness is not simply a matter of being “less strong”—it can influence nearly every physical task throughout the day.

Fatigue can make those physical challenges even more complicated. When muscles are already weak, the body may have to recruit more effort to accomplish relatively small movements, causing energy to be depleted faster. Repeating the same task several times may become increasingly difficult, and abilities can fluctuate depending on sleep, illness, respiratory health, nutrition, positioning, stress, and how physically demanding the day has been. Someone with SMA may be able to complete an activity once but struggle to repeat it later, which can be misunderstood as inconsistency or lack of effort. In reality, conserving energy can be an important part of managing SMA, and strategies such as mobility equipment, adaptive technology, caregiver assistance, rest periods, physical therapy, and proper positioning can help preserve function and reduce unnecessary exhaustion.
