Inside Evrysdi: The Exact Mechanism Protecting Your Muscles
Evrysdi, also known as risdiplam, does not directly strengthen or protect muscle fibers themselves. Instead, it works upstream by targeting the backup SMN2 gene, helping the body produce more functional SMN protein. According to the current FDA prescribing information, Evrysdi is an SMN2 splicing modifier that increases the inclusion of exon 7 in SMN2 messenger RNA, resulting in greater production of full-length SMN protein.
The mechanism behind Evrysdi is important because motor neurons depend on adequate SMN protein to survive and communicate effectively with muscles. Normally, the SMN2 gene produces relatively little full-length SMN protein because exon 7 is frequently excluded when its RNA is processed. Risdiplam changes that splicing process so exon 7 is included more often, allowing the body to create more complete SMN protein. FDA clinical data show that Evrysdi increased median SMN protein levels by more than twofold within four weeks of beginning treatment across the SMA populations studied. By supporting motor neurons in this way, Evrysdi helps preserve the nerve-to-muscle communication needed for movement and motor function rather than acting like a traditional muscle-building medication.
