How BIO101 May Boost Muscle Strength in SMA
BIO101, also known as 20-hydroxyecdysone, is an investigational drug attracting interest in SMA, because it targets muscle through a pathway that is different from current SMN-based treatments. Rather than increasing SMN protein, BIO101 activates the MAS receptor, part of a biological system involved in regulating skeletal muscle metabolism and function. Researchers believe this activation can stimulate pathways such as AKT that support muscle growth, protein synthesis, and muscle-cell development. This approach could be important because even when modern SMA treatments increase SMN protein and protect motor neurons, some individuals may continue experiencing muscle weakness, fatigue, and reduced physical function.
The strongest SMA evidence for BIO101 so far comes from laboratory and animal research. In a 2025 study, BIO101 reduced muscle atrophy in severe SMA mice, accelerated muscle-fiber maturation, improved neuromuscular-junction development, and produced measurable improvements in motor performance. Researchers also studied muscle cells derived from people with SMA and found that BIO101 increased myotube size and improved muscle-cell differentiation. Perhaps most interestingly, when BIO101 was combined with an experimental antisense therapy related to nusinersen, the SMA mice showed more than a threefold improvement in resistance to muscle fatigue compared with antisense treatment alone. These effects occurred without increasing SMN protein, suggesting that BIO101 could potentially complement therapies that target the genetic cause of SMA by addressing muscle weakness through a separate biological pathway.
