Can SMA Carriers Develop SMA Later in Life?
In most cases, the answer is no: an SMA carrier has one working copy and one non-working copy of the SMN1 gene, and carriers generally do not show signs or symptoms of SMA. Carriers do not have SMA, but they can pass the faulty gene to their children, which is why carrier education is so important for families, couples, and future parents.
Understanding the difference between being an SMA carrier and having SMA is essential. Spinal Muscular Atrophy is usually inherited in an autosomal recessive pattern, meaning a child typically must inherit two non-working copies of the SMN1 gene — one from each parent — to have the condition. If both parents are carriers, each pregnancy has a 25% chance of the child having SMA, a 50% chance of the child also being a carrier, and a 25% chance of the child being unaffected. Late-onset SMA, such as Type 4, can begin in adulthood, but that does not mean a carrier suddenly “turns into” someone with SMA; it usually means the person has the genetic condition itself, often with factors such as SMN2 copy number influencing severity and age of onset.
